Hourly Employee Benefits
We have chosen to provide the following employee benefits at Biaggi’s for the purpose of attracting the most desirable and qualified applicants and to encourage loyalty among our employees. Many of the benefits outlined are discretionary, and thus, Biaggi’s reserves the right to rescind or modify our benefits for the orderly and efficient fulfillment of the restaurant’s responsibilities. When using the term “Full Time” in our explanations, we are referring to : 30 Hours/Week or More

Eligibility Requirements

  • Employees are eligiblity to enroll in Biaggi's mini-med plan within 30 days of their hire date.

  • If employees fail to enroll within their 30 days of hire they may only enroll in the future if they have a *qualifying event.

  • Employees must work a minimum of 30 hours a week to maintain coverage. Hours will be reviewed on a quarterly basis. Employees are notified if coverage is cancelled.

  • Employees receive an enrollment form and brochure when hired.

*Qualifying events include but are not  limited to: Involuntary loss of previous coverage, family addition i.e., birth and/or marriage. Events must be reported within 30 days. Contact Human Resources for eligibility.

 Biaggi’s Ristorante Italiano, LLC 401(K) Plan & Trust

  • You will be eligible to participate in the Plan if you have completed a one (1) year Period of Service and have attained age 21.
  • If you are a veteran and are reemployed under Uniformed Services Employment and Reemployment Rights Act of 1994, your qualified military service may be considered service with us. If you may be affected by this law, contact your Human Resources Director for further details.
  • Employee’s become eligible to participate starting the 1st of January or July following twelve months of continuous employment.
  • If an employee fails to enroll when they first become eligible, they must wait to enroll until the next open enrollment period in June or December with benefits starting in July or January if eligibility requirements have been met.


All W-2s for 2013 were sent to the address we had on file as of January, 2014.  To allow for delivery time thru the post office, we will not reissue any forms until February 8.  If you do not receive your W-2 by that date, please print off the form below and submit it to the address supplied on the form.  All requests must be made in writing and include the $5 processing fee.  If your W-2 was returned to us, the fee will be refunded to you.
